- You will be asked for payment on this page as well. There is a one time charge of $6.50.
- You should receive your records within 5 business days of completing your request.

To request a copy of your X-rays please call the film library at (413) 233-1237.
Third-Party Records Requests:
- For third-parties, such as attorneys and insurance companies, needing to request a copy of a patient's medical records please fax your request to (615) 777-3517
